Due to an error on the sleeve of the Apollo 11 Proof Half Dollar set, the United States Mint has announced a replacement program for customers who have bought the set. If you have a set that you purchased from the Mint’s site or at their relation locations, you are encouraged to call 1-800-USA-MINT (872-6468) or TTY at 1-888-321-MINT (6468) to get your updated sleeve. The corrected sleeves are being provided free-of-charge.
The issue with the original sleeves has to do with the designer’s information on the obverse and reverse of the coins.
The back of the outside packaging sleeve incorrectly shows Gilroy Roberts as designing and engraving both the obverse (heads side) and reverse (tails side) of the Kennedy half dollar. Roberts designed and engraved the obverse, but Frank Gasparro designed and engraved the reverse.

Also, the replacement sleeves will have the signature of David Ryder, the current director of the Mint. The original sleeve was signed by David Croft, former acting deputy director.
The good news is that you will not have to return the entire set. Once you call the Mint, a replacement sleeve will be sent to you without having to send in anything. The Mint notes that this is a special exception as in previous cases like this, they have required the entire set to be return.
The proof commemorative is now very limited in availability. A check of the US Mint’s site showed that the coin was officially on back order, meaning few remain of the 400,000 that were to be made across all product sets. You can still signed up for the “Remind Me” function on the product page to get an email or text reminder should they become available once again.
